Description

Chemically, Ascorbic acid (Vitamin C) is (5R)-5-[(1S)-1, 2-Dihydroethyl]-3, 4-dihydroxyfuran-2-(5H)-one. Its molecular formula is C6H8O6 and its molecular weight is 176.1.

Thiamine hydrochloride (Vitamin B1) is 3-[(4-Amino-2-methyl pyrimidin-5-yl)-5-(2-hydroxyethyl)-4-methylthia-zolium chloride hydrochloride. Molecular formula is C12H18CI2N4OS and its molecular weight is 337.3.

Riboflavin (Vitamin B2) is 7,8-Dimethyl-10-[(2S,3S,4R) -2,3,4,5-tetrahydroxypentyl]benzo[g]pteridine-2,4(3H, 10H)-dione. Its molecular formula is C17H20N4O6, and its molecular weight is 376.4.

Pyridoxine hydrochloride (Vitamin B6) is (5-hydroxy-6-methylpyridine-3, 4-diyl) dimethanol hydrochloride. Molecular formula is C8H12CINO3, and its molecular weight is 205.6.

Cyanocobalamin (Vitamin B12) is alpha-(5,6-Dimethyl-ben zimidazol-1-yl) cobamide cyanide. Molecular formula is C63H88CoN14O14P and its molecular weight is 1355.

Calcium Pantothenate (Vitamin B5) is calcium bis[3-[[(2R)-2, 4-dihydroxy - 3, 3 - dimethylbutanoyl] amino] propanoate]. Molecular formula is C18H32CaN2O10 and its molecular weight is 476.5.

Molecular formula of Nicotinamide (pyridine-3-carboxamide) (Vitamin B3) is C6H6N2O and its molecular weight is 122.1.

Clinical Pharmacology

Ascorbic acid (Vitamin C): It is essential for collagen synthesis and tissue repair; involved in some oxidation-reduction reaction as well as other metabolic pathways such synthesis of carnitine, steroids and catecholamines; and conversion of folic acid to folinic acid.

Thiamine hydrochloride (Vitamin B1): Thiamine pyrophosphate, the active form of thiamine, is a coenzyme involved in carbohydrate metabolism and nerve conduction.

Riboflavin (Vitamin B2): It acts as coenzyme in oxidation-reduction reactions and is essential for activation of pyridoxine and conversion of tryptophan to niacin.

Pyridoxine hydrochloride (Vitamin B6): Pyridoxal phosphate, the active form of pyridoxine, is involved in several metabolic transformation of amino acid, carbohydrate, fat and is also required for haemoglobin synthesis.

Cyanocobalamin (Vitamin B12): Methylcobalamin and 5-deoxy-adenosyl-cobalamin are the active forms of cyanocobalamin which acts as coenzymes in nucleic acid synthesis, control folate metabolism in haematopoiesis and maintain the functions of myelin sheath.

Calcium D Pantothenate (Vitamin B5): It is vital for amino acid and fatty acid metabolism.

Nicotinamide (Vitamin B3): It inhibits the synthesis of cholesterol and triglycerides.

Pharmacokinetics

All ingredients are absorbed rapidly following oral administration. All are water soluble vitamins and are not stored to any significant extent, the excess quantity being excreted in urine.

Contraindication

Hypersensitivity to vitamin B, C and any components of the formulation.

Precaution

Since the formula contains vitamin C, diabetes and patients prone to recurrent renal caliculi (e.g. dialysis patients) should not take excessive doses for long period of time.

Adverse Effects

Large dose of vitamin C can lead to diarrhea and other GI disturbances. Thiamine may occasionally cause ana­ phylactic shock in breastfeeding. Drug Interaction

Iron: Vitamin C enhances iron absorption.

Oral Contraceptives: Concomitant use may increase contraceptive effect.

Warfarin: Vitamin C may reduce the anticoagulant effect of warfarin.

Ethanol: Ethanol inhibits the absorption of vitamin B1 and B12.

Amiodarone: Concomitant use with vitamin B6 may enhance amiodarone-induced photosensitivity reactions.

Levodopa: Vitamin B may decrease serum levels of levodopa, phenobarbital and phenytoin.
