Isosorbide dinitrate is used in the treatment of angina pectoris, for congestive heart failure.
Adult:
Angina : 30 - 120 mg daily in divided dose
Heart Failure : 40 - 60 mg daily in divided dose
The maximum daily dose is 240 mg in divided dose. Elderly: A lower dose may be given particularly if you have kidney or liver disease.

Isosorbide Dinitrate is nitrate vasodilator used in the treatment of angina pectoris. Chemically, it is [(3S,3aS, 6R,6aS)- 3 - nitrooxy - 2 , 3 , 3 a , 5 , 6 , 6 a -hexahydrofuro [3,2-b]furan-6-yl]nitrate. The molecular formula is C6H8N2O8 and its molecular weight is 236.14.
Isosorbide dinitrate relaxes vascular smooth muscle by formation of the free radical nitric oxide (NO). Nitric oxide activates guanylyl cyclase. Isosorbide dinitrate relaxes coronary arteries, thereby increasing blood circulation through ischaemic area.
Isosorbide dinitrate is readily absorbed following oral administration. It undergoes first pass metabolism mainly in the liver to conjugated metabolites which provide vasodilatory activity.
Hypersensitivity to isosorbide dinitrate, angle-closure glaucoma, head trauma or cerebral haemorrhage, severe anaemia.
